About the position

The Social Worker Level II position at MPA Behavioral Health Training Academy is a grant-funded role focused on providing psychosocial and social work services to patients with behavioral health needs. The position emphasizes patient-centered care, ensuring compliance with local, state, and federal regulations while supporting improved patient outcomes and experiences. The role involves conducting assessments, facilitating counseling sessions, and collaborating with treatment teams to develop comprehensive treatment plans.

Responsibilities

  • Screens and assesses patients for mental health and substance use disorders.
  • Facilitates patient engagement and follow-up care.
  • Conducts initial intake assessments and treatment plans for patients with behavioral health needs.
  • Engages clients in individual and/or group counseling sessions using evidence-based practice interventions.
  • Supports psychotropic medication management, focusing on treatment adherence monitoring and side effects.
  • Adheres to record-keeping in the EHR and other required systems, reports, and related correspondence.
  • Makes recommendations and facilitates changes to treatment plans based on documented progress of clients.
  • Interviews domestic violence victims to assess the need for services.
  • Identifies and develops working relationships with community-based agencies.
  • Facilitates referrals for clients to social services and community-based agencies.
  • Participates in weekly meetings and keeps supervisor apprised of client progress.
  • Escalates emergencies to supervisory and administrative staff as needed.
  • Attends and participates in clinical case conferences and mandatory trainings.
  • May assist in the provision of clinical services under supervision.

Requirements

  • Valid license as an LMSW issued by the NYSED.
  • Knowledge of child and/or adult development.
  • Familiarity with social services, educational, vocational, health, and family systems.

Nice-to-haves

  • Spanish or other secondary language skills.
