About the position

The Licensed Master Social Worker (LMSW) at Memorial Hermann Health System plays a crucial role in providing clinical social work and complex discharge planning for patients and their families facing psychosocial challenges. This position involves crisis intervention, mental health assessments, and the coordination of multidisciplinary discharge plans, ensuring that patients receive comprehensive care tailored to their needs. The LMSW works collaboratively with an interdisciplinary team to address the psychosocial aspects of patient care, contributing to the overall goal of delivering high-quality, efficient healthcare services.

Responsibilities

  • Assess patient's and family's psychosocial risk factors through evaluation of prior functioning levels and support systems.
  • Develop and carry out treatment plans using clinical social work diagnoses and interventions as part of a multidisciplinary team.
  • Intervene with patients and families regarding emotional, social, and financial consequences of illness or disability.
  • Provide counseling and intervention related to treatment decisions and end-of-life issues.
  • Participate in discharge planning activities for complex patients to ensure timely discharge and appropriate post-discharge care.
  • Communicate with clinical care team members regarding discharge planning status of referred patients.
  • Advocate for patient and family empowerment in making healthcare decisions and accessing services.
  • Educate patients, families, and physicians about post-acute care options and address issues of choice.

Requirements

  • Graduate of an accredited Master of Social Work program (MSW).
  • Current license as a Master Social Worker (LMSW) in the state of Texas required.
  • ACM certification from American Case Management Association (ACMA) preferred.
  • Field placement or internship in health services/health care provider experience.
  • Acute inpatient hospital social work experience preferred.
  • Effective oral and written communication skills.
  • Working knowledge of DSM V and ICD-10 manuals.
  • Demonstrates knowledge and skill in social work assessment and treatment of patients for mental health status and substance abuse screening.
  • Excellent therapeutic communication and negotiation skills.
  • Strong analytical skills.
  • Working knowledge and/or experience in utilization management, managed care, and payer issues.
  • Exposure and/or experience in pre-acute and post-acute care, as well as community resources.
  • Ability to work independently and develop collaborative relations with interdisciplinary teams.

Nice-to-haves

  • Experience in crisis intervention and mental health assessment.
  • Knowledge of community resources for patient support.
  • Experience in child protection and family dynamics.
