About the position
The Social Worker position at The New Jewish Home is a temporary full-time role focused on providing comprehensive support to older adults and their families. The Social Worker will work as part of an interdisciplinary team to facilitate discharge planning, emotional support, and care coordination, ensuring the well-being of vulnerable elderly and ill populations.
Responsibilities
- Work as part of an interdisciplinary team to provide comprehensive discharge planning.
- Provide emotional support to patients/residents and families.
- Complete MDS and psychosocial assessments.
- Coordinate and facilitate discharges and care planning.
- Complete all necessary documentation related to patient care.
- Facilitate team and family meetings regarding resident rights and well-being needs.
Requirements
- MSW from an accredited School of Social Work.
- Valid New York State License in Social Work.
- Experience and/or expressed interest in working with adult and geriatric populations.
- Experience in long-term care is preferable.
- Solid written, communication, and organizational skills.
