About the position
The Social Worker at CentraCare Health will be an integral part of the interdisciplinary team, responsible for evaluating and addressing the needs of patients across the healthcare continuum. This role emphasizes independent judgment and collaboration to ensure patient safety and welfare, while supporting Patient and Family Centered Care principles. The Social Worker will provide direct services, educate healthcare team members, and engage in community activities, working in a hybrid model that includes both remote and on-site responsibilities.
Responsibilities
- Evaluate and determine appropriate needs for patients across the healthcare continuum.
- Use independent judgment and collaboration to promote patient safety and welfare.
- Support Patient and Family Centered Care principles in all actions.
- Provide direct services or education to other healthcare team members.
- Proactively address patient needs through anticipatory judgment.
- Engage in professional activities that may require community involvement.
Requirements
- Bachelor's Degree in Social Work or related field from an accredited college.
- Current Licensed Social Worker (LSW) in Minnesota.
- Working understanding of medical terminology and electronic medical records; Epic experience preferred.
