The Social Worker at CentraCare Health will be an integral part of the interdisciplinary team, responsible for evaluating and addressing the needs of patients across the healthcare continuum. This role emphasizes independent judgment and collaboration to ensure patient safety and welfare, while supporting Patient and Family Centered Care principles. The Social Worker will provide direct services, educate healthcare team members, and engage in community activities, working in a hybrid model that includes both remote and on-site responsibilities.
