Responsibilities

• Planning for pre-admission and discharge
• Providing psycho-social assessment and completing relevant parts of the minimum data set
• Utilizing the minimum data set to guide the care plan
• Recognizing the worth and uniqueness of each individual

Requirements

• Licensed Social Worker (LSW) certification
• Experience in a healthcare or rehabilitation setting
• Strong communication and interpersonal skills
• Ability to work collaboratively in a team environment
• Knowledge of community resources and support systems
