About the position

The Social Worker position at Lighthouse Guild International, Inc. involves providing community-based adult day health care services to individuals with chronic medical conditions and vision impairments. The role requires collaboration with a multidisciplinary team to develop and implement treatment plans, conduct assessments, and provide counseling and advocacy for registrants and their families.

Responsibilities

  • Conduct interviews with prospective registrants and gather data for the admissions process.
  • Develop and implement social work treatment plans for all registrants, re-evaluating their psychosocial needs periodically.
  • Provide counseling services to registrants and families using individual, family, or group modalities.
  • Provide advocacy, concrete services, and information and referral as needed.
  • Track authorizations from managed care organizations and request new ones when necessary.
  • Lead topical discussion groups and therapeutic activity groups.
  • Develop discharge plans and make referrals or placements as needed.
  • Consult with other staff on psychosocial/mental health issues.
  • May supervise social work students and/or volunteers.
  • Ensure timely completion of all required records and reports.
  • Attend all required staff meetings and conferences.
  • Work collaboratively with other team members to foster a unified treatment approach.
  • Perform general assistance tasks as assigned by the program director, including escorting registrants, assisting with transportation, serving meals, and cleaning up.
  • Maintain confidentiality of registrant protected health information.
  • Follow the LGI workforce security policy; access information systems and ePHI authorized by clinical authorization only.
  • Observe and implement the Registrant's Bill of Rights.

Requirements

  • Master's degree in Social Work from an accredited program.
  • Minimum of two years of experience working with the elderly and/or disabled population.
  • Strong communication skills.
  • Proficient in Microsoft-based programs.
  • Good working knowledge of local community-based services, the NYS Medicaid System, and the DOH Universal Assessment system.
  • Experience in obtaining services through waivered and/or managed Medicaid plans.

Nice-to-haves

  • Prior experience in Long Term Care, Community Health, or Adult Day Health Care.
  • NYS LMSW licensure.
