About the position

The Social Worker (MSW) position at CHI Saint Joseph London is a full-time role focused on providing psychosocial support and care coordination for patients within the acute care setting. The Social Worker collaborates with healthcare professionals, patients, and families to ensure optimal clinical outcomes and effective resource utilization. This role involves conducting assessments, developing individualized care plans, and facilitating discharge planning, all while adhering to legal and ethical standards.

Responsibilities

  • Conducts psychosocial assessments on identified priority patients in collaboration with family.
  • Outlines available strategies and services to assist in addressing psychosocial care needs and makes recommendations as appropriate.
  • Utilizes specialized screening tools for assessments, including Depression, Anxiety, Substance Abuse, Health Literacy, Mental Status, and Social Support screens.
  • Develops individualized psychosocial case plans in collaboration with the Acute Care Case Manager and other Care Management team members.
  • Documents actions in the medical record according to departmental guidelines and oversees the exchange of information with other facilities/agencies while adhering to confidentiality mandates.
  • Provides crisis interventions and psychosocial counseling services to restore patients and families to optimal social and psychological health.
  • Provides factual information and materials to patients/families regarding programs and end-stage illnesses.
  • Makes referrals promptly to appropriate agencies for services suitable to meet patients' needs.
  • Screens patients for financial assistance and helps clarify insurance benefits, supporting patients and families in coping with economic stress due to serious illnesses.
  • Assists patients and families in enrollment in needed programs and services.
  • Assesses need for services through multidisciplinary rounds and collaboration with physicians and interdisciplinary team members.
  • Consults with outside agencies in the delivery of individual/community care necessary to meet identified goals.
  • Arranges and coordinates a broad spectrum of discharge planning services for patients during hospitalization.
  • Collaborates with patient/family, physicians, and care management staff to determine post-discharge needs and plans.
  • Provides patients/families with information on available options/resources and strategies for utilizing them.

Requirements

  • Master of Social Work (MSW) required.
  • Current unrestricted license as a social worker in KY (MSW:KY) required.
  • Demonstrated experience in case management, discharge planning, and transfer coordination.
  • Excellent computer skills and ability to learn new systems.
  • Knowledgeable regarding issues of chronic illnesses, loss/grief issues, family systems, and community services.
  • Strong organizational (time management) skills and interpersonal skills with attention to detail.
  • Proficient in business English, spelling, arithmetic, and effective communication skills, both written and verbal.
  • Basic knowledge of Microsoft Excel and Word, and ability to use information technology to evaluate care effectiveness.
