About the position

The position involves effective communication and collaboration with all members of the health care team to facilitate the episode of care, ensuring that patients receive services in a safe and timely manner. The role includes providing necessary resources and education to patients and their families. The candidate will interpret and communicate complex patient and family needs while role modeling caring practices to other caregivers. Responsibilities also include assessing patients' and families' psychosocial risk factors, counseling them regarding the emotional, social, and financial consequences of illness or disability, and mobilizing family and community resources to meet identified needs. The position requires providing mental health assessments upon physician referral and therapeutic intervention to facilitate complex discharge planning. The candidate will serve as the point person for issues related to child and adult abuse/neglect and will act as a liaison between Indiana University Health and state and county protective teams. Additionally, the role involves providing intervention in guardianship, foster care, adoption, and mental health placements.

Responsibilities

Requirements

  • Master's Degree in Social Work required.
  • Permanent LSW licensure required; LCSW licensure preferred.
  • Temporary LSW licensure considered with expectation of full LSW licensure within first year of employment.
  • Relevant health care, mental health, or community-based experience required.
  • Knowledge of the impact of disease processes and human behavior for psychosocial assessments and crisis intervention.
  • Knowledge of state and county adult and child protective legal mandates.
  • Knowledge of growth and development principles over the life span.
  • Proficiency in Microsoft Excel and Word.
  • 0-3 years of Medical Social Work experience required.

Nice-to-haves

  • De-escalation certification preferred.
