The Master of Social Work (MSW) will provide comprehensive psychosocial services to patients within our ambulatory care setting. This role involves addressing patients' psychosocial needs and enhancing their overall well-being. The Social Worker will play a crucial part in facilitating the adjustment of patients and their families to illness, ensuring optimal functioning for both individuals and families. The Social Worker will be expected to excel in various areas, including providing compassionate and evidence-based psychosocial support to children with various diagnoses, such as medical illness, ADHD, depression, anxiety, and autism spectrum disorders. Additionally, the Social Worker will assist families in navigating the healthcare system effectively, linking them with a range of services tailored to meet their unique needs. The position requires applying age-specific principles of growth and development to practice, collaborating seamlessly with all members of the healthcare team, and fostering effective communication and teamwork. The MSW will conduct assessments, collaborate with the multidisciplinary team, and ensure effective communication across disciplines to support patients and their families by addressing their biopsychosocial needs. The responsibilities include conducting thorough assessments of patients and their families' biopsychosocial situations, establishing clear goals, providing crisis intervention services, and assisting patients in accessing community resources. The Social Worker will also participate in case conferences and maintain accurate documentation in the Electronic Medical Record system, adhering to Medicaid, CMS, and other regulatory requirements.
