About the position

The Social Worker position within the PACT/GM & Specialty Clinics at the Department of Veterans Affairs focuses on providing outpatient social work services to Veterans receiving medical care. The role involves assessing and addressing the psychosocial needs of Veterans, collaborating with interdisciplinary treatment teams, and developing intervention plans to improve health outcomes.

Responsibilities

  • Provide outpatient social work services to Veterans in PACT clinics, specialty clinics, and emergency departments.
  • Conduct thorough assessments to identify emotional, social, physical, familial, economic, and vocational needs of Veterans.
  • Develop and implement patient-centered intervention plans using a strengths-based empowerment model.
  • Collaborate with interdisciplinary treatment teams and provide consultation regarding psychosocial needs of Veterans.
  • Identify and facilitate access to VHA/VBA/community resources for Veterans and their families.

Requirements

  • Master's degree in social work from an accredited school of social work.
  • Licensure or certification to independently practice social work at the master's degree level in the state of employment.
  • Proficiency in written and spoken English.
