About the position

The Behavioral Health Social Worker is a part-time position at Penn Medicine, focusing on providing clinical assessments and interventions for patients with comorbid medical, mental health, and substance use disorders. This role involves care management, navigation, and follow-up to ensure optimal clinical outcomes for at-risk patients, while also facilitating discharge planning and coordinating resources across various healthcare settings.

Responsibilities

  • Conduct biopsychosocial assessments with patients identified by providers, nurses, or requests from patients and families.
  • Administer suicide risk assessments for at-risk patients seeking behavioral health assistance.
  • Determine the least restrictive level of care in collaboration with the patient's treatment team.
  • Coordinate discharge plans and transfer plans based on care recommendations and resource needs.
  • Complete necessary psychiatric commitment paperwork and coordinate mental health court hearings as needed.
  • Assist colleagues in managing clinical crises as part of the Behavioral Response Team (BRT).
  • Serve as a behavioral health advocate and liaison with medical providers across the continuum of care.
  • Participate in hospital-based, health system-based, or community-based workgroups and committees.
  • Perform duties in accordance with Penn Medicine values, policies, and procedures.

Requirements

  • Masters of Social Work degree required.
  • PA license of Social Work (LSW) preferred.
