About the position
THEMA Health Services (Hospice) is seeking an extraordinary social worker with a Master of Social Work degree or Licensed Clinical Social Worker (LCSW) certification, who possesses a genuine passion for hospice care. This position is currently available as a PRN role, with the potential to transition into a full-time position as the company expands. We pride ourselves on having an exceptional team dedicated to providing compassionate hospice care to those in need. Previous experience in hospice care is a requirement for this role. The ideal candidate will either reside in Green Valley or be comfortable commuting from Tucson to Green Valley, as the job primarily involves working in Green Valley and surrounding areas. The social worker will be expected to travel to patients' homes, providing care wherever the patient calls home. Initially, this role will be compensated per visit rather than by the hour, with the listed pay rate being an approximation. However, full-time employment would entail a standard 40-hour work week. As a social worker in our hospice team, you will conduct assessments and evaluations of individuals to determine their needs and develop appropriate treatment plans. You will collaborate with other healthcare professionals, including nurses, physicians, chaplains, and educators, to create comprehensive treatment plans. Additionally, you will provide counseling and support to individuals and families during end-of-life situations, assist clients in accessing community resources and support services, and maintain accurate and up-to-date documentation of client interactions and progress.
