About the position

Grace 21 Hospice Agency LLC is seeking a compassionate Medical Social Worker to provide essential social services to patients in their homes, focusing on improving the quality of life for seniors. This role involves working under the supervision of the Director of Nursing to assess patients' psychosocial needs and develop care plans that address their emotional and social factors affecting health.

Responsibilities

• Provide medical social services to patients in their homes according to physician orders.
• Assist in the admission process by performing initial evaluations and assessing patients' psychosocial status.
• Develop and implement interdisciplinary patient care plans related to medical social work.
• Observe, record, and report changes in patients' emotional and social factors affecting their illness.
• Consult with attending physicians regarding alterations to treatment plans.
• Maintain and submit written clinical records, including evaluations and care plans.
• Evaluate patients' and families' responses to medical social work interventions.
• Confirm weekly visit schedules with the Director of Nursing to coordinate care.
• Participate in agency activities and committees, including staff development and in-service education.
• Supervise the social work assistant monthly.
• Participate in discharge planning.

Requirements

• Master's Degree from a School of Social Work approved by the Council of Social Work Education.
• Certification from the Academy of Certified Social Workers (or in the process of acquiring it).
• Two years of experience preferred, with at least one year in a healthcare setting.
• Current CPR certification.
• Criminal background check.
