About the position

The Social Worker in this position serves as the Coordinator for the Contract Nursing Home Program and Adult Day Health Care program within the Veterans Health Administration. This role involves managing both administrative and clinical aspects of the programs across multiple campuses, ensuring quality care for veterans, and developing policies and procedures to enhance program operations. The position requires collaboration with various stakeholders and oversight of program evaluations and resource utilization.

Responsibilities

  • Coordinate interdisciplinary staffing and work assignments for the program.
  • Provide direct clinical services to contract nursing home residents and address patient concerns.
  • Serve as the Coordinator for the Adult Day Health Care Program.
  • Collaborate with the Director of Care Management Service Line in policy development.
  • Track resource utilization and allocation decisions in collaboration with the ADHC Committee.
  • Ensure monthly follow-ups with veterans in the Contract Nursing Homes (CNHs) are completed as required.
  • Assure that placements into the CNH program meet VA eligibility requirements.
  • Complete annual VA CNH and program reviews as per VA policy.
  • Serve as the Contracting Officer Representative for the CNH program.
  • Ensure quality assurance related to the programs is maintained within parameters.
  • Report and investigate sentinel/adverse events as necessary and chair the CNH Oversight Committee.
  • Maintain a current list of active participants in the program for tracking service activities.
  • Provide administrative, technical, and educational consultation for program operations.
  • Resolve informal complaints and grievances related to the programs.
  • Review requests for authorization of veterans for the ADHC program and ensure eligibility criteria are met.
  • Provide oversight to ensure quality care is delivered by contracted ADHC providers.
  • Perform program analysis, development, review, and monitoring activities.

Requirements

  • Must be a U.S. Citizen.
  • Master's degree in social work from an accredited school of social work.
  • Current state licensure or certification to independently practice social work at the master's degree level.
  • One year of experience equivalent to the GS-11 grade level in social work.
  • Demonstrated advanced practice skills and judgment in social work.

Nice-to-haves

  • Certification or post-master's degree training from a recognized professional organization.
  • Experience in program coordination and administration.
