About the position

The Social Worker (Program Coordinator) serves as the Grant and Per Diem Program Liaison for the Fargo VA Health Care System, focusing on the Health Care for Homeless Veterans (HCHV) Program. This role involves coordinating and providing services to community-based programs funded under the VA's Homeless Providers Grant and Per Diem Program, ensuring effective communication between community providers and the National GPD Program Office. The position requires case management, oversight of funded programs, and direct services to veterans, with a strong emphasis on compliance and quality care delivery.

Responsibilities

  • Serve as the liaison/case manager for the HCHV Program's Homeless Providers Grant and Per Diem Program.
  • Monitor, coordinate, and provide services to community-based programs funded under the GPD Program.
  • Utilize various treatment modalities for individual and group work, casework, and community outreach activities.
  • Provide and monitor clinical care and assess compliance of programs receiving per diem funding.
  • Verify veteran status and eligibility for participation in the GPD Program.
  • Maintain administrative files for each GPD-funded program and coordinate annual program site inspections.
  • Advocate for veterans to ensure comprehensive service delivery and link them to VA Medical Centers and community agencies.
  • Engage with community agencies to learn about available services for homeless veterans and provide information on VA benefits.
  • Organize at least one Veterans Stand Down each year within the catchment area.
  • Participate in program planning, development, and performance improvement measures.

Requirements

  • Master's degree in social work from an accredited school of social work.
  • Licensed or certified by a state to independently practice social work at the master's degree level.
  • Proficient in spoken and written English.
  • One year of experience equivalent to the GS-11 grade level in social work, including knowledge of community resources and psychosocial assessments.
  • Demonstrated advanced practice skills and judgment in social work.
