About the position

The Social Worker (Program Coordinator) serves as the administrative head of the Homeless Patient Aligned Care Team (HPACT) within the Department of Veterans Affairs. This role involves developing policies and procedures for program operations, managing data related to program functioning, and preparing reports for various stakeholders. The incumbent is responsible for overseeing the quality of services provided to homeless veterans and ensuring effective program evaluation and improvement.

Responsibilities

  • Responsible for key project activities related to the planning, development, and implementation of the HPACT.
  • Coordinates team staff meetings and care huddles; oversees the HPACT consult process and clinic schedule.
  • Administers information and analytical systems to evaluate and enhance the quality of service provided to patients.
  • Develops and implements information systems to track program activities including referrals, admissions/discharges, patient demographics, and satisfaction.
  • Evaluates program progress and outcomes, utilizing data to continuously modify and improve the program and processes.
  • Uses sound judgment in interpreting trends and patterns, applying guidelines, and recommending program enhancements.
  • Performs specialized treatment of patients with complex mental and physical illnesses and homelessness.
  • Incorporates complex multiple causation in differential diagnosis and treatment of veterans.
  • Teaches and mentors staff and students in the area of practice and may provide supervision for licensure or specialty certifications.
  • Provides education to HPACT staff on changes in VA policy.
  • Collaborates with members of the HCHV team, GPD programs, and other clinics to provide comprehensive care to Veterans.
  • Conducts individual and family clinical interventions to identify substance abuse, psychiatric illness, and social needs.
  • Documents patient and family contacts in the Veteran's medical record via CPRS.
  • Participates in program planning, development, and evaluation.
  • Advises the section supervisor of shifting trends with caseloads and recommends program procedure or policy changes.
  • Collaborates with VA staff and community agencies to develop effective healthcare services for homeless Veterans.

Requirements

  • Master's degree in social work from an accredited school of social work.
  • Licensed or certified at the advanced practice level, able to provide supervision for licensure.
  • One year of experience equivalent to the GS-11 grade level, demonstrating advanced practice skills and judgment.
  • Knowledge of program coordination and administration, including consultation, negotiation, and monitoring.
  • Ability to write policies, procedures, and practice guidelines for the program.
  • Ability to supervise multidisciplinary staff assigned to the program.
  • Skill in organizing work, setting priorities, and meeting multiple deadlines.

Nice-to-haves

  • Certification or post-master's degree training from a nationally recognized professional organization or university.
  • Experience in working with homeless populations or veterans.
