About the position

The position involves providing assistance to patients and their families, as well as health care providers, to identify and facilitate access to tangible and supportive services related to patient health care needs. This includes updating the information resource directory and maintaining related literature and applications for distribution to staff and families in a specific area of concentration. The role also requires negotiating and advocating for patients' access to selected skilled, acute, or long-term care facilities, as well as identifying available transportation resources and negotiating with patients and families for cost-effective and safe transportation.

Requirements

  • Bachelor's degree in Social Work or Human Service field or equivalent education and experience required.
  • Master's degree in Social Work preferred.
  • Excellent interpersonal skills required.
  • Strong communication skills required.
  • Organizational skills required.
