About the position

The Social Worker at San Ysidro Health will provide essential support and advocacy for patients, focusing on the health, safety, and well-being of vulnerable and high-risk populations. This role involves proactive case management, psychosocial assessments, and connecting patients with necessary community resources to improve health outcomes.

Responsibilities

  • Provide proactive case management to improve health outcomes for SYHealth patients.
  • Conduct face-to-face psychosocial assessments and interventions for patients and families.
  • Perform in-depth screenings for community programs to educate and connect patients with needed services.
  • Obtain written authorization from patients and create profiles on the Community Information Exchange (CIE) system.
  • Utilize active listening to provide high-quality confidential connections to resources and services.
  • Identify programs and resources based on social determinants of health using the PRAPARE Tool.
  • Develop and implement case management plans in collaboration with patients and healthcare professionals.
  • Maintain ongoing patient caseload for continuity of care and assess barriers to care.
  • Document all patient interactions and unmet needs in NextGen to identify service gaps.
  • Comply with CIE regulations and complete all required trainings.
  • Maintain accurate records of activities and submit reports by due dates.
  • Represent SYHealth professionally and maintain positive relationships with clients and agencies.

Requirements

  • Bachelor's Degree in Social Sciences, Social Work, Psychology, Gerontology, Sociology, or related field.
  • CA driver's license and a vehicle with appropriate insurance coverage.
  • First Aid/CPR Certification (can be obtained within 6 months of hire).
  • At least 6 months of experience in a clinical healthcare setting or 1 year as a Case Manager.
  • Strong organizational and analytical skills.
  • Knowledge of community resources and their application to meet family needs.
  • Ability to establish and maintain good interpersonal relationships.
  • Excellent verbal and written communication skills.
