About The Position

The Long-Term Services and Supports (LTSS) area of Health and Human Services is seeking to fill multiple Senior Social Worker vacancies. The primary functions of these case manager positions are listed below. Each individual vacancy will have specialized duties and responsibilities depending on the area of assignment: Transition Team, Children's Long-Term Case Management Team, and Adult CADI/BI Case Management Team. These positions are hybrid and will be performed both on-site at a variety of Hennepin County Service Center locations, at community locations and residences to meet with people supported by LTSS programs, and remote as job duties require. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 4:30 p.m., with some flexibility after the probationary period. While these positions are designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. These are full-time, benefit earning positions. Candidates will be hired into one of these three areas: Transition Team, Children's Long-Term Case Management Team, or Adult CADI/BI Case Management Team.

Requirements

• Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles of social welfare.
• Bachelor's degree or higher in social work, psychology, sociology, or human services, and at least 18 semester (27 quarter) credits of graduate work in the above areas, and three years or more of full-time supervised experience as a social worker.
• Licensure as a Graduate Social Worker by the Minnesota Board of Social Work.
• A valid driver's license, the ability to obtain a Hennepin County driver's permit and daily access to reliable transportation sufficient to travel to a variety of Hennepin County community locations, to visit clients in residential settings, and to occasionally transport clients.

• Experience working with individuals with developmental and/or intellectual disabilities, physical disabilities, mental health needs, and medical conditions.
• Providing case management and/or completing MnCHOICES assessments.
• Using the Social Service Information System (SSIS), Electronic Case Files (ECF), and/or the Medicaid Management Information System (MMIS).
• Working with Microsoft Office (Microsoft Word, PowerPoint, Excel, OneNote, and Outlook).
• Knowledge of MnCHOICES Revision Assessments and Support Plans.
• Knowledge of available community, waiver, and state plan services.
• Knowledge of Employment First MN and its work with individuals, community members, and providers to promote employment opportunities for individuals with disabilities.
• Knowledge of social work best practices and ethics.
• Knowledge of issues related to equity and ability to demonstrate a commitment to promote diversity and inclusion.

Responsibilities

• Provide comprehensive case management with waiver and non-waiver individuals.
• Meet with individuals and other team members in the home or community to determine needs and strengths, identify goals, address crises/concerns, and to develop person-centered plans to support the individual's autonomy and integration in the community.
• Complete required documentation such as case notes, support plans, service agreements, and court reports, and perform annual review of eligibility determination.
• Build partnerships with persons served to identify and advocate for their needs while helping to reduce barriers or racial disparities while accessing services.
• Work collaboratively with internal and external partners and providers to facilitate the goals and services in individuals' best interest.
