About the position

The Social Worker - Vet Center Readjustment Counselor serves as a vital member of a multi-disciplinary team, providing essential counseling services, outreach, referral, and follow-up care coordination to eligible veterans and their families. This role focuses on establishing therapeutic relationships, conducting assessments, and developing individualized counseling plans to support the readjustment of veterans into civilian life.

Responsibilities

  • Screen and assess eligible individuals to establish therapeutic relationships and complete readjustment counseling intake procedures.
  • Develop and periodically update individualized readjustment counseling service plans that reflect therapeutic and psychosocial interventions.
  • Implement individualized counseling plans through individual, group, couple, and family counseling sessions.
  • Conduct risk assessments and crisis interventions, including developing safety plans for high-risk individuals.
  • Establish and maintain a network of community care relationships to coordinate access to readjustment counseling services.
  • Maintain active coordination with outreach workers to ensure seamless referrals to Vet Center services.
  • Participate in staff meetings to promote team building and staff development.
  • Document all clinical interactions and episodes of care coordination as required by policy.

Requirements

  • Must be a U.S. Citizen.
  • Master's degree in social work from an accredited school of social work.
  • Licensed or certified by a state to independently practice social work at the master's degree level.
  • Minimum of one year of post-MSW experience in health care or social work-related settings, or a doctoral degree in social work may substitute for experience.
  • Proficient in written and spoken English.

  • Experience working with veterans and understanding military culture.
  • Knowledge of VA benefits and services.
  • Ability to provide consultation services to new social workers and students.
