About the position

The Social Worker (Veteran Justice Outreach Program Coordinator) is responsible for establishing partnerships with correctional facilities to facilitate the identification and access of veterans to necessary services. This role involves coordinating care for veterans transitioning from incarceration to community living, ensuring they receive appropriate assessments, referrals, and support from both VA and non-VA resources. The position requires collaboration with various stakeholders, including legal representatives and community organizations, to develop comprehensive post-release plans for veterans.

Responsibilities

  • Establish informal or formal agreements and partnerships with city and county corrections regarding identification of veterans.
  • Meet with city and county jail administrators and staff regularly to maintain policies and procedures.
  • Attend correctional institution staff trainings relevant to working with offenders and high-risk populations.
  • Establish a point of contact at each facility for scheduled visitations with veterans.
  • Collaborate with county probation officers and legal representatives to develop post-release plans.
  • Establish contact in enrollment departments to expedite access to medical care for veterans.
  • Provide information on VA services to correctional institutions.
  • Attend relevant trainings, meetings, and conferences to enhance coordination of care for veterans.
  • Participate in state and community initiatives for homelessness prevention and veteran support.
  • Educate VA staff on first intercept point issues and activities.
  • Establish effective relationships with veterans and institutional staff.
  • Conduct pre-release needs assessments for incarcerated veterans.
  • Provide information on VA services and community resources to incarcerated veterans.
  • Coordinate veteran care with VA and non-VA programs.
  • Document assessments of psychosocial factors affecting veterans.
  • Advocate for veterans to ensure comprehensive service delivery and access to resources.
  • Follow up on the medical, psychiatric, social, and economic status of veterans during their transition.
  • Assist veterans in contacting legal representatives and navigating the judicial system.
  • Identify and intervene in crises experienced by veterans during community stabilization.

Requirements

  • U.S. Citizenship is required.
  • Master's degree in Social Work from an accredited school.
  • Licensed or certified to independently practice social work at the master's degree level.
  • Proficient in written and spoken English.
  • Ability to serve a probationary period and undergo a background/security investigation.
