About the position

Optum CA is seeking a Social Worker to join their team in Arcadia, CA. This role is integral to the Optum Care Delivery team, focusing on improving healthcare for patients through collaboration with various healthcare professionals. The Social Worker will assess patient and family psychosocial needs, develop care plans, and connect patients to community resources, all while advocating for continuity of care and supporting discharge planning.

Responsibilities

• Assess patient and family psychosocial needs and develop a plan of care in collaboration with the healthcare team.
• Link patients and families to appropriate community resources, including financial assistance and support groups.
• Provide inpatient support and facilitate discharge planning, assisting in the post-discharge phase.
• Recommend Doc to Doc consults for members exceeding appropriate length of stay and monitor admission days for conversion to administrative days.
• Apply brief solution-based therapy approaches and implement harm reduction strategies when appropriate.
• Document patient/family status, diagnosis, treatment plan, goals, interventions, evaluation results, observations, and progress in medical records.
• Serve as a patient advocate and liaison with physicians, families, insurance companies, and community agencies to ensure continuity of care.
• Provide discharge planning based on meeting identified goals of the treatment plan.

Requirements

• Bachelor's degree in social work (BSW) or equivalent human services.
• 1+ years of experience in a Social Work-related role.
