About the position
The Social Worker at AmeriCare Adult Day Health Care Center is responsible for assessing and planning care for clients with various medical and psychosocial issues. This role involves monitoring services and interventions to ensure quality care, acting as a liaison among participants, families, caregivers, and professionals, and engaging in community outreach efforts.
Responsibilities
- Conduct assessments and care planning for clients with multiple medical and psychosocial problems.
- Plan and monitor services and interventions to ensure quality care.
- Act as a liaison between participants, families, caregivers, and professionals.
- Participate in community outreach efforts.
Requirements
- Master's degree in Social Work, Psychology, Gerontology, or Counseling from an accredited school, or a Bachelor's degree in Social Work from an accredited school.
- Experience providing social services in aging, health, or long-term care services.
- Good communication and presentation skills.
- Demonstrated case management skills and experience in the community health care delivery system.
- Detail-oriented with good problem-solving skills and the ability to prioritize multiple tasks.
- Experience with community outreach.
- Computer literacy.
Nice-to-haves
- Bilingual in Spanish or Russian (preferred but not required).
- CBAS/ADHC experience.
