The position involves providing professional social work services as part of a multidisciplinary team at the Minnesota Veterans Home in Hastings. The incumbent will manage a caseload of residents, offering case management, counseling, referral, and discharge planning services. This role supports veterans with various medical, psychiatric, and chemical dependency diagnoses, ensuring they receive the necessary care and support in an independent living community.

Responsibilities

• Provide case management services for residents.
• Conduct counseling sessions with veterans.
• Facilitate referrals to appropriate services.
• Plan and coordinate discharge processes for residents.
• Travel between Hastings and Minneapolis Domiciliary Program locations.

Requirements

• Professional social work degree or equivalent experience.
• Experience in case management and counseling.
• Knowledge of medical, psychiatric, and chemical dependency issues.
