About the position

The Social Worker position within the Patient Aligned Care Team (PACT) at the VA Loma Linda Health Care System focuses on delivering comprehensive primary care to Veteran patients. The role emphasizes prevention, health promotion, care coordination, and chronic disease management, ensuring that healthcare services meet the unique needs of Veterans. The Social Worker acts as a care coordinator, collaborating with Veterans, their families, and interdisciplinary teams to develop and implement client-centered treatment plans.

Responsibilities

• Interact with Veterans, family members, and community health care providers to facilitate care.
• Participate as a member of the interdisciplinary treatment team to develop and implement treatment plans.
• Establish professional relationships with Veterans and engage in treatment.
• Utilize knowledge of medical and mental health diagnoses to inform treatment procedures.
• Serve as a liaison between Veterans and community resources to ensure service delivery.
• Provide consultation to treatment team members regarding psychosocial needs of Veterans.

Requirements

• U.S. Citizenship is required.
• Master's degree in social work (MSW) from an accredited school of social work.
• Licensure or certification to independently practice social work at the master's degree level is required for GS-11 level.
• Ability to work with diverse populations and assess psychosocial needs.
• Knowledge of medical and mental health diagnoses and treatment procedures.
