About the position

The Social Worker position at Catholic Medical Center involves providing psychiatric intake, individual and group therapy services as part of a psychiatric treatment team. The role focuses on delivering psychotherapeutic and substance misuse services to adult patients, ensuring quality care and advocating for patient needs. The Social Worker will work under the supervision of the Medical Director and Practice Director, engaging in patient assessments, treatment planning, and collaboration with interdisciplinary teams.

Responsibilities

  • Perform psycho-social assessments/social histories on new admissions within established time frames.
  • Establish written treatment and discharge plans in collaboration with patients, families, and treatment teams.
  • Plan and conduct individual and group therapy sessions.
  • Monitor patient's progress to ensure appropriate and timely care.
  • Advocate for patients to ensure quality outcomes.
  • Provide supportive counseling and crisis intervention for patients and families as needed.
  • Participate in the development, implementation, and evaluation of patient care plans.
  • Identify high-risk populations and work proactively with Department Coordinators to ensure daily schedule capacity.
  • Respect the dignity and confidentiality of patients and all communication.
  • Review patient intake information within 24 hours to determine appropriateness of care level.
  • Assess patient's clinical course in collaboration with the treatment team and revise plans as needed.
  • Provide third-party payors with concurrent review information to comply with documentation requirements.
  • Explore strategies to reduce length of stay and resource consumption while promoting optimal patient outcomes.
  • Address financial concerns impacting patient care and provide clinical information to third-party payors.
  • Interpret federal and state regulations regarding coverage issues and educate patients and families.
  • Conduct assigned quality management activities to improve patient care and service.
  • Participate in the development or revision of departmental policies/procedures related to patient care processes.
  • Assist in identifying organizational priorities for improvement and collect clinical data for quality management.
  • Present aggregated clinical quality data to team members in a timely manner.
  • Facilitate multidisciplinary meetings to review treatment issues and outcomes.
  • Monitor patient's status to identify appropriate level of care for timely discharge planning.
  • Perform chart reviews for early identification of discharge planning needs and facilitate planning with the interdisciplinary team.
  • Assess patient/family adaptation to illness/treatment for discharge planning.
  • Coordinate outreach to patients who no-show or cancel appointments.

Requirements

  • Master's Degree in Social Work or Mental Health Counseling from an accredited program required.
  • LICSW (Licensed Individual Clinical Social Worker) or LCMHC (Licensed Clinical Mental Health Counselor) required.
  • Minimum of three years' experience in outpatient Behavioral Health preferred.

Nice-to-haves

  • Licensed Alcohol and Drug Abuse Counselor (LADAC) preferred.
