About the position

The Outreach Coordinator will be responsible for coordinating outreach programs and engaging the community to promote the mission of the South Asian Center for Diabetes & Heart Health. This role focuses on educating the South Asian community about metabolic diseases, providing resources for prevention, and connecting families and caregivers. The position offers a hybrid work environment, combining remote work with in-person responsibilities.

Responsibilities

  • Develop and implement outreach strategies to increase community awareness.
  • Provide support to community members and guide them towards reliable sources of information pertaining to their metabolic health.
  • Help with compiling information to respond to informational queries from the community.
  • Support recruiting new members and encourage participation in SACDHH outreach events.
  • Support outreach activities such as tabling at events, sharing informational flyers and brochures.
  • Partake in any necessary professional development to complete above duties.
  • Ability to perform the Rapid Risk Test in the screening Clinics.
  • Organize educational workshops and events.
  • Conduct data collection and analysis to measure outreach effectiveness.
  • Perform other additional duties to support the program as required.

Requirements

  • Familiarity with providing support through face to face, phone and email modalities.
  • Knowledge of Public Health & community education.
  • Health background with an ability to perform phlebotomy for the Rapid Risk Test.
  • Knowledge about care coordination and health education.
  • Basic computer skills (Google Docs/Sheets and Microsoft Word/Excel) for making flyers, creating registration forms, activity tracking, organizing resources, etc.

Nice-to-haves

  • Experience in social media management.
