About The Position

The Activity Coordinator at Miracle Adult Day Health Care (ADHC) plays a crucial role in enhancing the lives of individuals with developmental disabilities by providing structured and therapeutic recreational activities. This part-time position focuses on developing personalized activity programs that cater to the needs and interests of participants, ensuring their social and emotional skills are nurtured in a supportive environment.

Requirements

• Completed at least 36 hours of training in a course designed for this position and approved by the California Department of Aging.
• Experience in patient activities program or professional certification as an occupational therapist, art therapist, music therapist, dance therapist, recreation therapist, occupational therapy assistant, or qualified Social Worker.
• Bilingual in English and Spanish is required.
• Basic computer skills.

Responsibilities

• Develop activity programs that meet the needs and interests of center participants.
• Consult with participants to determine activities that align with their wants, needs, and goals.
• Conduct initial and six-month assessments, as well as quarterly progress notes.
• Perform reassessments after hospitalization or changes in health status.
• Evaluate participants' mental and functional status related to activities.
• Develop person-centered treatment plans that include problems, goals, and treatments.
• Record, date, and sign treatment as activity services are provided on the Activities flowsheet or narrative notes.
• Act as a member of the Multidisciplinary team and participate in MDT meetings.
