About the position
The Hospice Admissions RN at Heart of Hospice is responsible for managing the hospice admission process, ensuring compliance with federal and state regulations while advocating for patients and their families. This role emphasizes building trusting relationships and providing compassionate end-of-life care, supported by a strong team and leadership. The position requires effective communication with various stakeholders to facilitate patient admissions and meet their needs.
Responsibilities
- Execute the hospice admission process in compliance with federal and state regulations.
- Advocate for patients and families to meet their needs and refer them to appropriate resources.
- Demonstrate competency with potential patients and assess hospice eligibility.
- Maintain confidentiality and safety of patient/client clinical records.
- Complete assigned admissions and patient care orders effectively and timely.
- Communicate effectively with physicians, referral sources, patients, families, and the hospice interdisciplinary team regarding new admissions.
- Record relevant information to determine hospice eligibility.
- Communicate with operational, clinical, and sales leadership regarding the status of pending referrals.
Requirements
- Must be a licensed registered nurse in the state of operation.
- At least 2 years of experience in a hospice, home health, or other health care service delivery system setting.
- Current CPR certification.
- Current Driver's License and vehicle insurance, and access to a dependable vehicle or public transportation.
- A minimum of two years full-time experience working as an RN in Louisiana, or two years of full-time clinical experience in hospice care as a licensed practical nurse may substitute for the required experience.
