About the position

The Care Manager for the Emergency Department is responsible for overseeing daily care management activities in the Ambulatory/Community Setting. This role involves collaboration with medical staff, nursing, and clinical leaders to ensure optimal patient care, financial goals, and leadership development. The position includes accountability for ambulatory care management services, which may encompass medical social work, transition planning, and care coordination. The Care Manager will engage in tactical, operational, and strategic duties to enhance care management programs and meet organizational goals.

Responsibilities

• Coordinate care and services for patients needing assistance or meeting Care Management criteria.
• Collect in-depth information about patients' medical, functional, and social conditions to develop tailored care plans.
• Work with patients and families to establish specific goals and actions based on assessments.
• Coordinate discharge planning and execute interventions to meet established goals.
• Organize and integrate necessary resources to accomplish care goals.
• Assess patients' prior functioning levels and access to community resources.
• Conduct psychosocial needs assessments and interventions as per guidelines.
• Support behavioral health processes based on site guidelines.
• Assist the care team in developing comprehensive care plans, ensuring appropriateness of services and care settings.
• Collaborate with physicians and healthcare team members on behalf of patients.
• Identify admission and continued stay issues to enhance patient care quality.
• Help identify strategies for reducing length of stay and optimizing service utilization.
• Utilize clinical pathway data to implement strategies for resolving variances.
• Participate in rounds and case conferences, advocating for patient rights.

Requirements

• Master's degree in Social Work from an accredited program (for Social Worker role).
• LCSW license or SWC/LSW required at hire, with LCSW required within 3 years of employment.
• Three years of experience as a Care Manager in an acute care setting (for Social Worker role).
• Bachelor's Degree in Nursing (BSN) required (for RN role).
• Current valid State RN License required (for RN role).
• Three years of experience as a Registered Nurse in an acute care setting (for RN role).

Nice-to-haves

• At least five years of care management experience in an acute care setting.
• Care Management Certification.
