About the position

The Care Transition Coordinator (CTC) is responsible for executing the sales strategy to enhance the company's market share by developing accounts and educating the medical community about the services offered. The CTC facilitates seamless transitions for patients moving from facility settings to home care, ensuring that all patient needs are met and documented. This role involves collaboration with facility discharge planners, managing sales administration duties, and driving growth through effective communication and education of both patients and healthcare providers.

Responsibilities

  • Facilitate seamless transitions for patients discharging from facilities to home care.
  • Develop accounts and educate the medical community on company services.
  • Verify receipt of home health orders and assess agency's ability to meet patient needs.
  • Achieve monthly Personal Production Goals and manage sales and marketing expenses.
  • Execute strategies to increase market share within assigned facilities.
  • Evaluate patient suitability for home care and initiate face-to-face transitions.
  • Educate patients on home care services and coordinate transfer orders and ancillary services.
  • Document patient needs and ensure they are met by the agency.
  • Drive growth by focusing on the needs of the referral community and patients.
  • Manage all sales administration duties including expense entry compliance and payroll.
  • Educate patients on the importance of follow-up appointments and medication understanding.
  • Serve as a liaison between the agency and healthcare providers for newly referred patients.
  • Provide feedback to case management regarding readmissions and non-admit decisions.

Requirements

  • One year of home health experience or one year of hospital case management experience.
  • Current RN, LPN, SW, or PT licensure in the state of practice.
  • RT and/or technical school certification demonstrating strong clinical knowledge.

Nice-to-haves

  • One to three years of medical marketing experience preferred.
