About the position

The Caregiver position at Commonwealth Community Based Services involves providing in-home personal care and health-related services to patients, ensuring their physical and emotional well-being. This role emphasizes building trusted relationships with clients while adhering to care plans and agency policies. The position is PRN, allowing for flexible scheduling and the opportunity to earn paid time off (PTO) while working 32+ hours.

Responsibilities

  • Provide in-home personal care and designated health-related services to patients.
  • Follow the written plan of care and adhere to Medicare/Medicaid regulations and agency policies.
  • Report observations of the client's condition to the Agency Director or accounts manager.
  • Notify the Agency Director immediately of any incidents or accidents.
  • Utilize the Electronic Visit Verification (E.V.V.) program as applicable.
  • Drive safely and comply with traffic regulations when business travel is required.
  • Assist clients with activities of daily living, including bathing, grooming, and dressing.
  • Provide medication reminders and assist with household tasks essential to personal care.
  • Accompany clients to medical appointments or shopping as necessary.
  • Measure and record vital signs when ordered, within training limitations.
  • Prepare meals based on clients' preferred diets and assist with feeding if required.
  • Maintain a clean and safe environment within the home, ensuring safe use of equipment.
  • Document all client-related activities in a timely manner and respond promptly to client requests.

Requirements

  • Six months of experience in home care preferred.
  • Current CPR certification required.
  • Ability to work flexible hours to meet patients' needs.
  • Ability to function in any home situation regardless of the patient's background.
  • Ability to work independently and manage time effectively.
  • Good communication, writing, and organizational skills.
  • Strong commitment to the organization's goals, mission, and philosophy.

Nice-to-haves

  • Experience with Electronic Visit Verification (E.V.V.) program.
  • Familiarity with home health care regulations and best practices.
