About The Position

The Case Manager at Dignity Health East Valley Rehabilitation Hospital plays a crucial role in providing comprehensive care to patients in a state-of-the-art rehabilitation facility. This position involves collaborating with an interdisciplinary team to ensure effective patient care delivery, addressing barriers to care, and facilitating communication with patients and their families throughout the rehabilitation process.

Requirements

  • Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
  • Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, RN-BC).
  • Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
  • Six months to a year of experience with Cerner or similar EHR system.
  • Technical proficiency and ability to manage multiple software/programs systems (Cerner, RightFax, UDS Rehab Connect).
  • Effective oral and written communication skills in English; additional languages preferred.
  • Basic computer skills in Excel, Word, Outlook, PowerPoint, etc. required.

Responsibilities

  • Completes departmental orientation, initial and annual competencies.
  • Consults with other departmental staff to collaborate in patient care delivery, identify barriers to care, and develop solutions.
  • Completes documentation per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines.
  • Schedules family conferences and communicates with caregivers to keep them informed of patient progress and rehabilitation plans.
  • Coordinates weekly patient care team conferences to develop, monitor, and refine treatment plans to achieve patient goals.
  • Reviews the patient's assigned CMG and identifies any potential missed comorbid conditions, communicating findings to the HIM team.
  • Performs other duties as assigned.
