About the position

The Field RN, Care Manager position at Adventist Health involves delivering coordinated nursing care to patients in their homes. The role requires supervising nursing staff, coordinating care with other healthcare disciplines, and utilizing critical thinking and professional judgment to ensure high standards of care are met.

Responsibilities

  • Deliver coordinated nursing care for patients in their place of residence according to established standards of care.
  • Supervise and direct the activities of various levels of assigned nursing staff.
  • Coordinate care with other disciplines while utilizing critical thinking and professional discretion.
  • Collect relevant data pertinent to the patient's health or situation.
  • Analyze assessment data to determine diagnosis and care issues.
  • Develop a care plan that prescribes interventions to attain desired outcomes.
  • Implement the care plan and coordinate care delivery.
  • Evaluate progress toward attaining health outcomes.
  • Collaborate with patients, families, and healthcare providers to provide safe and humane patient care.
  • Provide learning opportunities for patients and family members.
  • Assess and document patient/family learning needs and understanding.
  • Complete all Oasis documentation accurately and completely.
  • Delegate appropriately and coordinate duties of healthcare team members.

Requirements

  • Bachelor's Degree in Nursing (BSN) preferred.
  • One year of professional nursing experience required.
  • Acute care facility or home health experience preferred.
  • Registered Nurse (RN) licensure in the state of practice required.
  • Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification required.
  • Valid Driver's License (DL) and must be at least 21 years of age or older required.

Nice-to-haves

  • Experience in home health care settings.
  • Additional certifications in nursing specialties.
