About the position

The Health Coordinator plays a vital role in the Community Living department, ensuring that individuals receive the highest quality of healthcare. This position involves scheduling and coordinating medical appointments, maintaining accurate health documentation, and advocating for individuals' health needs. The Health Coordinator is responsible for ensuring compliance with physician orders and medication management, while also facilitating communication among healthcare providers, individuals, and their families.

Responsibilities

  • Ensure medical appointments are scheduled and completed.
  • Advocate for individuals' healthcare needs with physicians and staff.
  • Document medical appointments, treatment plans, and health-related goals according to policy.
  • Track annual examinations and routine lab work, as well as unexpected medical needs.
  • Enter all appointments into the Therap calendar to minimize conflicts.
  • Provide detailed information for monthly RN reviews in Therap.
  • Review doctors' orders and request necessary documentation from pharmacies.
  • Ensure accuracy of new medications against doctor's orders and MARs.
  • Prepare documentation for medication destruction when necessary.
  • Check MARs for compliance and documentation regularly.
  • Create new MARs for new individuals admitted into residential services.
  • Maintain and scan medical records into Therap with current information.
  • Request medical reports from hospitals for complete medical records.
  • Ensure physician's orders are signed annually and report any concerns to the nurse or physician.
  • Review and arrange updates for individual immunization records.
  • Maintain ongoing communication with healthcare providers and agency staff.
  • Participate in staff meetings to address health QA assessments and corrective actions.
  • Provide education regarding health matters to staff as needed.
  • Attend monthly health team meetings and assist other coordinators when available.

Requirements

  • High School diploma or General Equivalency Diploma (GED).
  • At least 1 year of successful employment in a similar work environment, or a combination of medical education and work experience equaling 3 years.
  • Training and certification in required areas such as Abuse/Neglect, First Aid, CPR, Level 1 Medication Administration, and Positive Behavior Supports.
  • Class E driver's license and minimum auto insurance coverage.

Nice-to-haves

  • Experience in healthcare coordination or similar roles.
  • Familiarity with Therap software for health documentation.
  • Strong communication and advocacy skills.
