About the position

The Home Health Aide (HHA) - Hospice Aide provides essential personal care and supportive services to patients receiving hospice care at home. This role involves working under the supervision of a registered nurse and requires effective communication with the case manager to ensure comprehensive patient care. The HHA is responsible for assisting patients with daily living activities, monitoring vital signs, and providing companionship, all while maintaining the dignity and respect of the patients and their families.

Responsibilities

• Provides in-home personal care and supportive services to Hospice patients.
• Assists patients with bathing, grooming, and personal hygiene.
• Monitors vital signs and reports changes to the case manager.
• Communicates with patients and families regarding scheduled visits and care plans.
• Maintains a safe and clean environment for patients.
• Completes documentation and synchronizes electronic devices after patient visits.
• Participates in interdisciplinary group meetings and educational trainings.
• Mentors new clinical providers and assists with their orientation.

Requirements

• Current California Home Health Aide Certification and CNA Certification required.
• Interest and ability to work with terminally ill patients.
• Recent experience providing personal care to patients/clients preferred.
• Effective communication skills in English, both written and spoken.
• Current California driver's license and automobile insurance.

Nice-to-haves

• Ability to commute to Sunnyvale, CA.
• Willingness to travel up to 25%.
