About the position
The Hospice Chaplain - Pastoral/Spiritual Counselor role at AdvisaCare involves providing spiritual and pastoral support to patients and their families in hospice care. This part-time position requires compassionate professionals with hospice experience to assess spiritual needs, develop care plans, and support bereavement efforts, ensuring compliance with hospice policies and maintaining high customer satisfaction.
Responsibilities
- Conduct spiritual and pastoral assessments of patients, families, and caregivers.
- Develop individualized spiritual plans of care in collaboration with local clergy as desired by the patient.
- Ensure compliance with AdvisaCare Hospice policies and Medicare regulations.
- Prepare clinical and progress notes and participate in interdisciplinary group (IDG) conferences.
- Provide bereavement support to staff and families.
- Participate in the hospice performance improvement program.
- Maintain high levels of customer satisfaction with clients, families, and caregivers.
- Consistently update Electronic Medical Records.
Requirements
- Graduate of an accredited seminary or school of theology or appropriate certification in pastoral ministry.
- Demonstrated experience working with patients and families dealing with loss.
- Empathetic approach to patients, caregivers, and team members.
- Ability to adapt to patients' status and needs.
- Exhibit critical thinking skills for continuous improvement of hospice services.
