About the position

The Hospice Chaplain provides spiritual and emotional support to hospice patients, their families, and staff at Village Hospice. This part-time role involves conducting spiritual assessments, developing care plans, and coordinating support groups, all while adhering to hospice policies and regulatory requirements. The chaplain plays a crucial role in ensuring that the spiritual needs of patients and families are met, particularly during end-of-life care.

Responsibilities

  • Provides emotional/spiritual support to hospice patients and family members/caregivers.
  • Completes initial spiritual assessment within 5 days of patient admission.
  • Documents spiritual assessments and develops spiritual care plans.
  • Attends weekly IDT meetings for hospice patients and facility Plan of Care meetings.
  • Maintains contact with community clergy regarding patients' spiritual needs.
  • Alerts RN Case Manager of patient changes/needs.
  • Documents pastoral care visits to patients and families/caregivers.
  • Participates in Village Hospice on-call support staff rotation.
  • Assists clients and families with health care, grief, and bereavement planning.
  • Conducts or assists in funeral arrangements or memorial services when requested.
  • Provides consultation and education to other team members as needed.
  • Assists in training/orienting volunteers, new associates, students, and interns in hospice philosophy related to spiritual needs.
  • Continues professional growth by attending seminars, workshops, or classes.

Requirements

  • Masters of Science degree in religion studies.
  • Advanced training in counseling, Clinical Pastoral Education, or Gerontology required.
  • Minimum of one unit Clinical Pastoral Education (CPE).
  • Ordained, commissioned, or credentialed according to the practice of an organized religious group.
  • One year of experience in hospice care required.
  • Valid driver's license and proof of current auto insurance.

Nice-to-haves

  • Training in common spiritual issues in death and dying.
  • Knowledge of belief systems of comparative religions related to death and dying.
  • Spiritual assessment skills and individualizing care to patient beliefs.
