About the position

The Hospice Nursing Assistant (STNA) role at Hospice of the Western Reserve involves providing personal care to patients under the supervision of a nurse. The position emphasizes collaboration with a transdisciplinary team to enhance the quality of life for patients in hospice care, ensuring comfort and dignity during end-of-life care. The role requires flexibility in meeting patient and family needs, including variable shifts and weekends, and involves various responsibilities related to patient care and documentation.

Responsibilities

  • Provide personal care and assist with Activities of Daily Living (ADLs) according to the Plan of Care (POC).
  • Ensure a safe environment for patients.
  • Safely ambulate and transfer patients as appropriate.
  • Assist with passive and active Range of Motion (ROM) exercises as directed by the nurse.
  • Prepare nutritional snacks for patients as appropriate.
  • Assist patients with pre-poured medications that are self-administered and prescribed by a physician under nurse supervision (Home Care only).
  • Perform simple wound care, skin care, and bowel care protocols as directed by the nurse.
  • Recognize and report any changes in the mental or physical status of the patient to the nurse promptly.
  • Document activities provided and observations of the patient in clinical records at the time of visit.
  • Complete all charting, timesheets, and mileage logs as per agency policy.
  • Attend and contribute to patient care conferences and team meetings.
  • Participate in in-services to obtain required education hours.
  • Participate in the on-call rotation as scheduled (Home Care only).
  • Assist the Team Leader with scheduling, supply organization, and answering office telephone as needed.
  • Demonstrate positive customer service responsiveness according to agency standards.
  • Participate in quality improvement activities and chart reviews as assigned.
  • Provide basic caregiving education to family members, such as turning and positioning, transfer techniques, and changing linens.
  • Maintain Transmission Based Precautions and complete annual competency requirements.

Requirements

  • Must be a state tested nursing assistant in good standing.
  • Must be able to read and write English proficiently.
  • Compliance with Annual Flu Shot Policy or ability to provide exemption documentation.
  • Provide an active auto insurance policy as a licensed driver in Ohio with the required coverage.
  • Possess an active driver's license.
  • Ability to provide proof of eligibility to work in the United States.

  • Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.
