About the position
The Hospice RN Case Manager at Community Loving Care Hospice in Wentzville, MO, is responsible for delivering comprehensive nursing care to hospice patients. This role emphasizes building trusting relationships with patients and their families, ensuring high-quality end-of-life care, and collaborating with a multidisciplinary team to meet the holistic needs of patients. The position requires continuous evaluation of care plans and personal performance to enhance productivity and care quality.
Responsibilities
- Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identify and reassess the patient's/family's physical, psychosocial, and environmental needs at least every 15 days.
- Initiate and coordinate the plan of care while maintaining accurate clinical notes regarding the patient's condition.
- Document problems, appropriate goals, interventions, and patient/family responses to hospice care.
- Collaborate with the patient/family, attending physician, and other members of the interdisciplinary group (IDG) in providing care.
Requirements
- Current RN licensure in the state of practice.
- One year of clinical experience.
- Current CPR Certification.
- Current driver's license and valid vehicle insurance, or access to public transportation.
