About the position

The Life Coach/Peer Coach position at Progress House Association is focused on providing trauma-informed support to individuals transitioning from the Lynnwood Jail back into the community. This role emphasizes personal development and community healing through comprehensive case management and restorative practices. The ideal candidate will leverage their lived experience to guide participants in their reentry process, ensuring they receive the necessary resources and support for successful reintegration.

Responsibilities

  • Provide support pre- and post-release inside and outside the jail.
  • Foster personal development and guide participants through their reentry process.
  • Utilize the Human Dignity Model, Restorative Circles, and the Wheel of Wellness in daily practices.
  • Develop tailored wellness plans addressing physical, emotional, spiritual, and mental health needs.
  • Conduct screenings and assessments using tools like CAIS, DAST-10, ACEs, PHQ-9, and GAD-7.
  • Develop comprehensive, trauma-informed, and personalized case management plans.
  • Facilitate connections to substance use and behavioral health treatment, housing referrals, employment assistance, and educational opportunities.
  • Assist participants in obtaining and managing state healthcare coverage.
  • Encourage engagement with faith-based and community support networks.
  • Participate in restorative circle training and assist in the preparation and facilitation of sessions.
  • Conduct proactive recruitment sessions inside the jail and engage with correctional officers to identify potential participants.
  • Actively involve community members in the project to ensure responsiveness and relevance.
  • Find and build a client roster.
  • Research resources and build relationships with community partners.
  • Schedule clients and share calendar.
  • Connect clients to resources and provide after-hours/on-call support.
  • Provide regular life coaching sessions covering various topics with clients.
  • Maintain accurate daily case notes and conduct in-person one-on-one and small group sessions multiple times a week.
  • Be available for client calls, meetings, etc., on weekends and on-call as agreed.
  • Oversee and facilitate support groups, documenting key takeaways and milestones.
  • Monitor and document participants' progress using program-specific tools.
  • Conduct regular wellness check-ins and update progress reports.
  • Maintain accurate and confidential records, including attendance, milestones, and case notes.
  • Serve as the liaison between participants, community organizations, and family members to optimize support networks.
  • Participate in community meetings, presentations, and relationship-building efforts with other organizations and employers.

Requirements

  • Experience in reentry support, case management, or related fields.
  • Lived experience is preferred.
  • Strong understanding of the challenges faced by individuals transitioning from incarceration.
  • Training or certification in restorative practices and trauma-informed care is preferred.
  • Excellent interpersonal and communication skills.
  • Ability to develop and maintain relationships with community partners.
  • Ability to drive client participation and build client roster.
