About the position

The Licensed Practical Nurse (LPN) at StoneBridge Jacksonville is responsible for providing essential medical care and support to residents, particularly the elderly, as part of a dedicated healthcare team. This role emphasizes compassion, patience, and a commitment to enhancing the well-being of residents through personalized care. The LPN will coordinate nursing care, administer medications, and perform various medical treatments while ensuring a safe and supportive environment for both residents and staff.

Responsibilities

  • Coordinates the nursing care of residents on the unit, ensuring all residents are assigned a capable nurse aide.
  • Uses independent judgment and discretion in performing nursing duties.
  • Provides safe and accurate medication-related interventions to residents.
  • Administers and documents medications and treatments according to each resident's medication schedule.
  • Understands the rationale for the effects of medications and treatments and administers them correctly.
  • Provides treatments per physician orders, including wound care and catheterization.
  • Orders and receives medications from the pharmacy.
  • Conducts accurate accounting of all narcotics and manages medication cart keys.
  • Responds promptly to resident requests for pain intervention and documents accordingly.
  • Transcribes and coordinates the implementation of physician orders accurately.
  • Performs nursing assessments regarding the health status of residents and notifies physicians of changes.
  • Prepares work assignment sheets considering resident safety and staff qualifications.
  • Reports staff shortages to nursing management.
  • Participates in employee actions that may result in reward or discipline.

Requirements

  • Must have a current state license to practice as a licensed practical nurse (LPN).
  • Current CPR certification and additional certification in a nursing specialty desired.
  • Demonstrates knowledge of basic nursing practice with an interest in gerontological nursing.
  • Prefer 2 years' experience in a long-term care setting.
  • Basic computer skills and experience with Electronic Medical Records preferred.
