About The Position

The Nurse Practitioner (NP) position at DaVita Integrated Kidney Care (IKC) in Tulsa, OK, is a full-time role focused on delivering holistic and integrated care to complex kidney patients. The NP will work autonomously and collaboratively with healthcare teams to improve patient outcomes and quality of life. This hybrid-remote position involves travel within the assigned geography, including dialysis clinics and nephrology practices, and offers a flexible schedule without nights, weekends, or on-call duties.

Requirements

• Minimum of two (2) years' experience as a Nurse Practitioner (NP).
• Current unrestricted NP license to practice in the state.
• Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire.
• Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.
• Intermediate computer skills and proficiency in MS Word, Excel & Outlook required.
• Home Office, High Speed Internet and Reliable Transportation required.

Nice To Haves

• Preferred experience in Value Based Care or Population Health.
• 5+ years' experience as a Family Nurse Practitioner (FNP) or in Internal Medicine, Cardiology, Nephrology, or Case Management.
• Spanish-speaking preferred in some markets.
• Experience in managing chronic diseases (CHF, diabetes, hypertension).

• Complete comprehensive health assessments.
• Manage complex patients to reduce hospitalizations and improve health outcomes.
• Coordinate and facilitate quality, consistent, cost-effective care.
• Work autonomously and in collaboration with healthcare team members.
