About the position

The Older Adult Clinical Services Specialist/Mental Health position is a vital role within the Multnomah County Health Department, focusing on providing comprehensive mental health services to older adults and individuals with disabilities. This position involves working collaboratively with a multidisciplinary team to bridge gaps in mental health and support services, ensuring that clients receive the necessary care and resources to live with dignity and independence. The role includes care coordination, community outreach, and the development of training programs to enhance service delivery for this population.

Responsibilities

  • Provide care coordination and collaboration between systems such as Aging, Disability and Veterans Services (ADVS), Mental Health, Alcohol and Drug, and Intellectual & Developmental Disabilities.
  • Maintain partnerships with agencies, healthcare providers, and service providers that work with older adults and people with disabilities.
  • Consult on transitions and discharges from Long Term Care, jails, and hospitals as needed.
  • Develop and promote training opportunities for community providers and families regarding mental health resources for older adults.
  • Identify access and referral information for existing services in the community and maintain a database of resources.
  • Document data and track outcome-based elements for the Older Adult Behavioral Health Initiative.

Requirements

  • Must meet the standards of the Qualified Mental Health Professional (QMHP-R) with the Mental Health & Addiction Certification Board of Oregon.
  • Graduate degree in psychology, social work, counseling, or a behavioral science field.
  • Driver's License required.
  • Background investigation may include fingerprinting.
