About the position

The Prescription Assistance Coordinator plays a crucial role in helping patients access necessary medications by assisting them in applying for prescription assistance programs. This position acts as a liaison between patients, healthcare providers, and pharmaceutical companies, ensuring that patients receive the support they need to navigate the application process and advocate for their medication needs.

Responsibilities

  • Serve as the primary point of contact for pharmaceutical companies and organizations offering prescription assistance programs.
  • Receive shipments of medications, verify accuracy, and maintain records of all deliveries.
  • Document received medications in the EMR system (NextGen) and notify patients of their arrival.
  • Identify patients in need of prescription assistance through screenings and referrals.
  • Educate patients about available prescription assistance programs and application procedures.
  • Assist patients in completing application forms and gathering required documentation.
  • Advocate for patients with pharmaceutical companies to expedite application processing.
  • Communicate with patients and healthcare providers to coordinate the enrollment process and address concerns.
  • Follow up with patients to track application statuses and resolve issues.
  • Maintain accurate records of patients' medication needs and applications in compliance with privacy regulations.
  • Collaborate with healthcare professionals to address social determinants of health affecting medication access.
  • Stay informed about changes in prescription assistance programs and eligibility criteria.
  • Participate in training sessions and quality improvement initiatives.
  • Identify community resources for medication disposal and take-back programs.
  • Perform additional responsibilities as needed to assist in the operation of CGMHSA.

Requirements

  • High School Diploma or Technical Certificate in a related field.
  • At least one year of experience in social services or healthcare focusing on assisting individuals with healthcare resources.
  • Knowledge of prescription assistance programs and healthcare systems.
  • Strong communication and interpersonal skills to engage with diverse patients.
  • Excellent organizational skills and attention to detail.
  • Empathy and compassion for serving vulnerable populations.
  • Proficiency in computer skills, including data entry and document management.
