About the position
The PRN Home Health RN position at Your Tailor Made Senior Service involves providing quality nursing care to patients in their homes, particularly focusing on adult home health assignments in Fort Worth and surrounding areas. The role requires collaboration with physicians and interdisciplinary teams to develop and implement care plans, ensuring compliance with healthcare regulations and policies.
Responsibilities
- Provide quality, comprehensive nursing care in the patient's home as prescribed by the physician.
- Ensure quality and safe delivery of home health care services in compliance with applicable laws and regulations.
- Perform nursing assessments and develop, implement, and evaluate plans of care in conjunction with the physician, patient, and caregivers.
- Evaluate the effectiveness of the plan of care and revise it as needed based on the patient's response and outcomes.
- Initiate appropriate preventive and rehabilitative nursing procedures following the physician's plan of care.
- Document all communications with the patient, family, physician, and other relevant parties.
- Prepare a written plan for the Home Health Aide to follow, if applicable.
- Collaborate with the interdisciplinary team to develop the plan of care for the patient.
- Manage nursing treatments and activities to meet the needs of each individual patient.
- Evaluate utilization of patient services and patient progress, reviewing evaluations with the care team, patient, family members, physician, and case manager as needed.
Requirements
- Valid RN license in Texas
- OASIS/Medicare Certified Visits Experience
- Electronic Charting experience (using Finale Health)
- Minimum of 1 year of recent home health nursing experience
- Driver's License and Vehicle with Current Auto Insurance
- American Heart Association BLS certification
- Problem-solving skills
- Excellent communication skills
- Positive, professional attitude
