Responsibilities

• Manage care planning and coordination for patients.
• Conduct health assessments and monitor the quality of services.
• Track progress towards individual patient goals.
• Oversee transitions for patients, including discharge planning from hospitals or nursing homes.
• Identify health, environmental, and psychosocial risks and recommend interventions.
• Collaborate with the Interdisciplinary Care team to implement care strategies.

Requirements

• Valid NYS RN License.
• Minimum of 3 years of relevant experience in Managed Care, Home Care, or Community settings.
• Associate's degree or higher in a relevant field.
• Exceptional communication skills and strong problem-solving abilities.
• Bilingual proficiency in languages such as Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole.
• Ability to navigate and utilize various software and tools relevant to care management.
