About the position

The RN Case Manager in Hospice at Assured Home Health & Hospice is responsible for delivering comprehensive nursing care to hospice patients, ensuring their comfort and dignity during end-of-life care. This role emphasizes building trusting relationships with patients and their families, while also collaborating with a multidisciplinary team to create and implement individualized care plans. The position requires continuous evaluation of care quality and productivity, making necessary adjustments to enhance patient outcomes.

Responsibilities

• Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Identify and reassess the patient's/family's physical, psychosocial, and environmental needs at least every 15 days.
• Initiate and coordinate the plan of care while maintaining accurate clinical notes regarding the patient's condition.
• Document problems, appropriate goals, interventions, and patient/family responses to hospice care.
• Collaborate with the patient/family, attending physician, and other members of the interdisciplinary group (IDG) in providing daily care.

Requirements

• Current RN licensure in the state of practice.
• One year of clinical experience.
• Current CPR Certification.
• Current driver's license and valid vehicle insurance.
• Access to a dependable vehicle or public transportation.
