About the position

As an RN Case Manager at Moments Hospice, you will be instrumental in providing compassionate care to patients during their final days, weeks, and months. This full-time position emphasizes personal and professional growth, offering a supportive work environment with manageable caseloads and on-call support. You will collaborate closely with the healthcare team and the company's owners, contributing to both patient care and the entrepreneurial spirit of the organization.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.

Requirements

• Licensed as a Registered Nurse / RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Nice-to-haves

• Nursing experience of 1 year (Preferred)
• Case management experience of 1 year (Preferred)
• Spanish language proficiency (Required)
