About the position

The RN Case Manager Supervisor is responsible for overseeing the integration of case management and social services for clients participating in a waiver program in Kentucky. This role involves leading a collaborative process to assess, plan, implement, coordinate, monitor, and evaluate services to meet clients' healthcare needs. The supervisor acts as a patient advocate, manages staff education, and ensures high-quality, cost-effective patient care.

Responsibilities

  • Coordinate the integration of Case Management and Social Services functions for clients in a waiver program.
  • Lead a collaborative process to assess, plan, implement, coordinate, monitor, and evaluate available services.
  • Act as a patient advocate and perform staff education related to resource utilization and discharge planning.
  • Manage and supervise to ensure high-quality, cost-effective patient care and services.
  • Oversee referral coordination and ensure patient needs are continually assessed and coordinated.
  • Develop, implement, adhere to, and update the patient's individualized plan of care.
  • Ensure effective communication with physicians, families, and other clinicians involved in care.
  • Educate staff through orientation, case conferences, and mandatory in-services.
  • Ensure qualified personnel are hired and meet licensure and certification requirements.
  • Provide in-home visits as needed.
  • Oversee accurate completion of clinical medical record documentation, including electronic visit verification.
  • Assist with complaint investigation, resolution, and follow-up as assigned.
  • Provide oversight to the agency's infection control plan and practice.
  • Serve as the Emergency Management Coordinator in the absence of the Executive Director.
  • Participate in the organization's Performance Improvement and Compliance Programs.

Requirements

  • Bachelor's or master's degree in a health and human service field.
  • At least 1 year of experience in a health and human service field or educational equivalent in aging or disability.
  • Master's degree in health or human services field is acceptable.
